Community Living Parry Sound (CLPS) has been working with people with developmental disabilities and their families for over 60 years, supporting people to build better lives in their community. As our programs and services continue to grow, we are seeking the perfect addition to our support services team. The ideal Individualized Funding Coordinator is energized by the CLPS mission and passionate about working with people and their families. The successful candidate will help to drive our mission forward by coordinating individualized intake and leave processes, supporting person-directed planning, and ensuring the highest standards of service.
POSITION RESPONSIBILITIES:
- Supporting intake and leave processs with individuals receiving service ensuring each person’s goals, preferences, and needs are central to all planning and facilitation.
- Review all applications for service, determine eligibility, and assess support needs.
- Facilitate person-directed planning using person-centered strategies to create and develop individualized support plan(s).
- Supporting with reports, tracking of budgets, spreadsheets, and monitor service delivery outcomes.
- Assist with marketing and promotion of individualized services, including supporting the development and delivery of individualized services or workshops and contributing to the integration of social and recreational opportunities aligned with person-directed goals.
- Collaborating with community partners to support inclusive opportunities for people with developmental disabilities.
REQUIRED QUALIFICATIONS:
- Actively working towards a Post-Secondary Diploma in Human Services, Social Services, Business Administration, or equivalent field.
- Experience working with people with developmental disabilities or a vulnerable population.
- Excellent interpersonal communication, with a professional and enthusiastic demeanour.
- Strong organizational and time-management skills, with the ability to work independently and as part of a team.
- Ability to effectively support complex or intensive planning processes with individuals and families.
- Proficiency with Microsoft Office and comfortable with technology database systems.
- Access to a reliable vehicle, Ontario G Driver’s licence for business use with a minimum $1,000,000 liability coverage
PREFERRED QUALIFICATIONS:
- Experience with intake case coordination, or person-directed planning.
- Familiarity with government-funded developmental services and reporting requirements.
- Valid First Aid/CPR Level C certification.
- Valid Non-Violent Crisis Intervention certification.
